For disability support workers

Tap what happened. Get clear NDIS shift notes.

A few taps become clear shift notes (progress notes) built around the person you support and their goals, ready to paste into your provider's system.

Some staff paste notes into general AI tools, which sends participant details to AI companies. Swift Notes removes known names on your phone before anything is sent, and mostly doesn't use AI at all.

Providers and families set it up once. Workers just tap.

Built around the person

Built around your participant, not a template.

Each participant gets their own buttons, set up from how their days really go. Upload their past shift notes and Swift Notes reads every one of them to suggest their buttons, then you check each one. Here are two example participants side by side.

How it works

From your shift to finished notes in three steps.

1

Tap what happened

Big, clear buttons for the things your participant actually does, with their usual choices already picked.

2

Check the wording

Clear, person-centred support notes, written in the order things happened. Change anything with a tap.

3

Copy and paste

One button copies the whole shift, hour by hour, ready for your provider's system.

Live demo

Meet Sam. Write his notes.

Sam is an example participant. This is the real app, running right here. Nothing you enter is kept or sent anywhere.

  1. 1Tap Outing, pick Bus and a few other things, then Add to shift.
  2. 2Tap Food & drink. Sam's usual choices are already selected.
  3. 3Tap Review shift notes to see the whole shift, ending with the goals Sam worked on.

With Swift Notes

Sam requested Vegemite toast and a glass of water. Staff provided verbal prompting with the meal. Sam travelled by bus with staff. Staff supported Sam to access the community; he visited the beach. He wore his noise-cancelling headphones. Staff supported Sam to stay active; he went to the gym for about 45 minutes. Staff supported Sam to build his independent living skills; he put a load of washing on and hung the washing out.

Why it matters

Notes that tell the whole story at plan review time.

When it's time for a plan review, clear and consistent records help show what support someone really needs and how they're going. Swift Notes makes that kind of record the easy default, every shift.

  • The same structure, every shiftNo more one-line notes after a long day. Give every worker a consistent, structured way to record support.
  • The support behind each momentPrompting levels, safety support and choices are written in, not left out.
  • Change you can point toTrends for sleep, meals, outings and prompting show what's changing over weeks and months.

Funding decisions are made by the NDIA, and Swift Notes can't guarantee any outcome. What it can do is make sure the day-to-day evidence is clear and complete.

Language mode

Tap in your language. The notes come out in English.

Many great support workers speak English as a second language. Writing notes in it at the end of a long shift is hard. With language mode, workers tap through the shift in the language they think in, and Swift Notes writes the same clear English note every time.

  • Menus in Nepali, Hindi, Punjabi, Filipino, Mandarin, Cantonese, Arabic or Vietnamese, chosen by each worker on their own phone. More languages coming soon
  • Same buttons, same order, so the whole team works the same way
  • Notes are written in English, ready for your provider's system
  • Workers can read a finished note in their language to check it says what they meant

The participant's own buttons are translated when the owner turns on writing help.

Goals

Their goals, in every shift.

Add the participant's support plan and Swift Notes picks out their goals and links them to the buttons that count. The team sees the goals at the start of each shift, and the notes show which goals came up, based only on what staff tapped.

  • A short reminder of the goals at the start of each shift. Just the list, never instructions
  • "Activities linked to goals" at the end of the notes, only for goals that came up
  • Trends show how often each goal came up over the last month
  • Only the goals are kept. Swift Notes doesn't keep the plan
Sam's goals
  • Ask for what he needs using his device
  • Catch the bus and train with less support
  • Take part in making simple meals
End of the shift notes

Activities linked to goals (from what staff recorded; not a progress assessment):
• Catch the bus and train with less support: Bus (11am)
• Take part in making simple meals: Helped make it, Washed hands (9am)

Our story

Built by a support worker, for support workers.

Hi, I'm Alex, the founder of Swift Notes. I've worked in disability support for nine years, and support workers already have enough on their plates without spending another 20 minutes at the end of every shift writing progress notes.

But saving time is only half the story. Good shift notes are an important record of a participant's daily life, their achievements, their challenges and the support they need.

Questions

Good to know

How does paying work? Is there a free trial?

You pay A$29 when you set up a participant, then the same each month. Instead of a free trial, there's a 30-day money-back: cancel within 30 days of your first payment and it's refunded in full, automatically, no questions asked. After that you can cancel any time and keep using it until the end of the month you've paid for. See the refund policy.

What happens to the notes if we stop?

Nothing is deleted straight away. The team can still read and copy every note for 12 months; only writing new notes is paused. Restart the plan any time in those 12 months to carry on, export a copy, or ask us to delete everything sooner.

Shift notes, progress notes, case notes: does it do all of them?

Yes. Providers call them different things, but they're the same daily record of the supports you gave. Swift Notes writes them hour by hour in clear, person-centred language, so they fit whatever your provider calls them.

Who is Swift Notes for?

Disability support workers, the families who coordinate their supports, and small providers. It was built by a support worker to write the notes support workers write every shift.

Does it replace my provider's system?

No. Swift Notes writes the note; you copy it into the system your provider already uses. That keeps your provider's records exactly where they expect them.

Where is information stored?

With a team account, notes are stored securely in Australia (Sydney) and shared only with the people the owner invites. The demo and the on-this-phone version keep everything in the browser on that device. See our privacy policy for the details.

Can it be set up for my participant?

Yes. Each participant gets their own buttons, places, foods, words and routines, so notes sound like their day, not a template. The owner can upload past shift notes and Swift Notes suggests the buttons from them, including what usually happens together and at what time of day. Nothing is used until the owner has checked it, and it's optional: you can set it up by hand instead.

Will better notes help with NDIS funding?

We can't promise that. Funding decisions are made by the NDIA based on many things. But plan reviews rely on evidence, and clear, consistent shift notes are part of that picture: what support was given, how much prompting was needed and how someone is going over time. Swift Notes is designed to make that record complete, every shift.

Can support workers use it in another language?

Yes. Each worker can switch the app's menus to Nepali, Hindi, Punjabi, Filipino, Mandarin, Cantonese, Arabic or Vietnamese on their own phone. With writing help turned on, the participant's own buttons show in their language too, and they can read a finished note in their language to check it says what they meant. The notes themselves are always written in English, ready for your provider's system.

Who writes the notes?

Staff tap what happened, and Swift Notes writes the sentences from the participant's own buttons, in the participant's own words and routines. Staff check everything before it's copied. Two optional extras use AI from Anthropic: reading past notes when you first set up, and writing help (a quick check of finished notes, and translation for workers who use another language). Writing help stays off unless the owner turns it on, and nothing is used to train AI. See our privacy policy for the details.
